Relationship Assessment Questionnaire
Both people in a relationship complete it separately. When both are done, a detailed report appears on the quality of the relationship — strengths, differences in perspective, and suggestions for both. It explores nine aspects of the relationship, based on qualitative assessment tools and attachment theory (the theory that describes how our earliest relationships with caregivers shape internal "models" of how we connect, trust, and feel safe; it is so important because these early patterns continue to influence how we love, seek closeness, and react to distance in our relationships as adults).
How it works
- 1The first person completes the questions.
- 2Once they finish, a code appears — they forward it to the second person.
- 3The second person chooses "I was given a code," enters it on their device, and continues filling it in.
- 4When both have finished, the shared report appears — for both, using the same code.
Completing it is anonymous — we don't ask for names, and the answers are linked only to a random code. Email is optional, used only to send you the report, and is not kept afterward. This is a self-observation tool, not a diagnosis.
